Description:

Molar Incisor Hypomineralization (MIH) is a clinically challenging condition affecting as many as 20-40% of children’s newly erupted permanent teeth. MIH may affect one or all of the molars, as well as the incisors. It is characterized by demarcated opacities ranging from white and cream to yellow and brown, sometimes surrounded by normal enamel. Sadly, these teeth suffer a 10-fold higher risk of developing caries, and the high porosity and low mechanical resistance lead to rapid enamel breakdown, sometimes occurring immediately after the eruption. MIH affected teeth are often hypersensitive, difficult to numb, and have poor bond strength to resin restoratives. Fortunately, the development of new materials offers promising new treatment options that are minimally invasive and well-tolerated by patients. This program reviews emerging clinical treatments like Silver Diamine Fluoride (SDF), Atraumatic Restorative Treatment (ART) with glass ionomer cement (GIC), Hall Technique, MI Paste, etch bleach seal, and Icon Resin Infiltration to manage MIH.

There is increasing demand from parents for minimally invasive treatment options for the restoration of dental caries in young children. An evidence-based, minimally invasive treatment option for the management of severe early childhood caries is the Hall technique (HT). This approach is particularly advantageous for young apprehensive patients, who may otherwise require sedation for traditional surgical interventions to place stainless steel crowns (SSCs). An emerging treatment modality that expands on the HT concept of sealing carious lesions from cariogenic biofilm is atraumatic restorative treatment (ART) using a glass hybrid restorative system and strip crown forms. This offers a non-invasive yet esthetic treatment alternative to prefabricated cosmetic crowns in young children with severely decayed primary incisors. This hands-on workshop walks participants through the step-by-step clinical placement of HT SSCs and glass ionomer cement (GIC) ART strip crowns. Attendees increase their confidence and mastery of these minimal interventions, helping them expand access to care while reducing cost and risk for pediatric patients.

Early Childhood Caries (ECC) is the most widespread chronic disease in children worldwide but is entirely preventable. There is a need to focus on minimally invasive ECC prevention and emphasize primary prevention before caries occur. The foundation of oral health is established during the first years of a child’s life and includes a mandatory age-one visit and establishment of a dental home; prioritizing non-invasive restoration strategies; treatment plans that put the patient first; and motivational interviewing, anticipatory guidance, and working with families to set self-management goals.

Children and teens are more affected socially and emotionally by the pandemic than adults. The psychological toll on young people is enormous. This burden is manifested by an increase in bruxing. The role that stress plays in bruxing cannot be denied. Recognizing and treating anxiety will be addressed. One of the most common treatments provided for bruxing and TMD is an oral appliance. Understanding how to select the right appliance is discussed as well as appliances used for mixed dentition and orthodontic treatment. Most importantly there are many other treatments, in addition to appliances, that will be considered.

MIDAZOLAM: CONSCIOUS SEDATION IN PEDIATRIC DENTISTRY 

Pediatric dental procedures performed in the operatory often rely on excellent clinical skills and equally practical behavior guidance. Encompassed within “behavior guidance” exists many different pharmacologic and non-pharmacologic techniques ranging from Tell- Show-Do and Distraction to conscious and IV sedation. This program explores one of the more commonly used aware sedation medications in pediatric dentistry, Midazolam. We dive into its history of use, pharmacological properties, and practical application in pediatric dentistry.
